Ischemic risk

Arrhythmia risk

Postoperative angina

Acute infarction within 6 weeks

Left ventricular ejection fraction (LVEF) <35%

Active ischemia by angina or exercise testing

NYHA grade III or IV congestive heart failure (CHF)

Significant left ventricular dysfunction (LVEF <30%)

Ventricular tachycardia or fibrillation in the postoperative period

History of sustained ventricular tachycardia

The systolic blood pressure drop of 10 mmHg or more with exercise

History of sustained life-threatening supraventricular arrhythmia

Incapable of self-monitoring

History of sudden death

Myocardial ischemia with exercise

Initial therapy of patients with automatic implantable cardioverter-defibrillator (AICD)

Initial therapy of a patient with a rate-adaptive cardiac pacemaker
